Medical Coverage Opt-Out Bonus Opt-Out Form	If you are eligible for medical coverage through UM Health-Sparrow and also have medical coverage through a company other than UM Health-Sparrow Health System (which includes all wholly-owned Affiliates), you may choose to opt-out of your UM Heath-Sparrow health insurance.	MNA-Represented Home Care Caregivers who opt out of health insurance and complete an Opt-Out form are eligible to receive a $75.00 monthly opt-out bonus.	No changes
