Sparrow Provider Network
Use the available search criteria to find providers within the Sparrow Provider Network. 
Be sure to choose “UMH-SPN (team members only)” in the “Network/Plan Type” field under “I’m a Guest” on the right.

BlueCross BlueShield
Choose “Find Care” for “Individuals and families.”  Scroll down to “Find a Doctor” and choose “search without logging in.” Click on “Choose a location” and enter the requested information, following the prompts. Next in the upper righthand corner click on Plan “I don’t know my network” and then click “Find a different plan.” In the plan name search field type “Sparrow” and choose Sparrow Hospital as the selected plan; Confirm selection. Then you may choose from the search options provided, or utilize the Advanced Search option.

Delta Dental
Choose “Find a Dentist.” Scroll down and be sure to choose “Search EPO” for the Bronze plan or “Search PPO and Premier” for the Silver (Base) and Gold (Buy-Up) plans.  Use the available search criteria to find providers within the chosen network.

VSP
Under Find a Doctor, click “Search Now.” On the next page search by location after entering a zip code, address, city and/or state.
It is possible to narrow the results by searching for vision doctor under a specific specialty or by entering the name of a vision provider to confirm they are in-network under our employer plan.
