|Short-Term Disability Insurance (STD)||Provides a % of a Caregiver’s base monthly earnings in the event s/he becomes disabled and unable to work for a short period of time. Maximum limits may apply – please contact HR for more details.||Voluntary – must be purchased by MNA Caregivers during their new hire eligibility period, within 30 days of a qualified status change or during the Annual Open Enrollment period. Caregivers who sign up for coverage after their initial enrollment period will no longer be required to complete an evidence of insurability (EOI) to gain coverage. Instead, all Caregivers who elect coverage will gain coverage but will be subject to a pre-existing condition clause. Under this clause, MNA Caregivers that sign up would not be covered for pre-existing conditions only if they received treatment or took prescriptions drugs for the condition three (3) months prior to the first 12 months of the policy coverage.

The cost of the base long-term disability insurance plan is paid by the Employer. Hourly employees will be given an option of purchasing a plan with a 60-day elimination period. The cost to the employee will be the difference between the base long-term disability insurance plan and the 60-day plan. Salary employees will have the opportunity to enhance their LTD benefit with a Buy-Up option that increases coverage to 66 2/3% of earnings up to the maximum benefit.
