Available to you and your immediate family members within your household.	Enhanced and expanded EAP offerings: Now includes up to 8 free counseling sessions per person, per issue, per calendar year; New expanded offerings now include Foundations Computerized Behavioral Therapy and WellthSource Financial Wellness tools.
Medical Coverage Opt-Out Bonus Opt-Out Form	If you are eligible for medical coverage through Sparrow and also have medical coverage through a company other than Sparrow Health System (which includes all wholly-owned Affiliates), you may choose to opt-out of your Sparrow health insurance.	MNA-Represented Home Care Caregivers who opt out of health insurance and complete an Opt-Out form are eligible to receive a $75.00 monthly opt-out bonus.	No changes
