Choose “Find a Doctor” and then “search without logging in.” Click on “Choose a location” and enter the requested information, following the prompts. In the upper righthand corner click on “All Plans” then click “Find a different plan.” In the Available Plans: Employer Group Plans section, scroll down and choose Sparrow Hospital in the “Hospital Blue Plans” section; Confirm selection. Then you may choose from the search options provided, or utilize the Advanced Search option.

Caregivers have access to both PPO and Premier networks for the Base and Buy-Up plans. When you’re in the search screen, you can select: PPO only, Premier only or PPO Plus Premier, or EPO.
